What this confirms — and what it doesn't
An active NPI and license mean the provider is registered and authorized to practice under that credential — it isn't a quality rating, and it doesn't reflect malpractice history, patient reviews, or board disciplinary actions (check your state medical board for those). Practice locations and specialties are self-reported by the provider and may be out of date.
